Final programme report · Community health

Community HIV prevention, testing and linkage to care

Programme evidence from Yumbe District and Bidibidi Refugee Settlement on community testing, counselling, referral, retention and service integration.

Abstract

Community services linked people back to care.

The report assesses a community-based programme designed to expand HIV prevention, testing, counselling and linkage services in a mobile humanitarian setting. Community Linkage and Retention Assistants, outreach teams, peer educators and health-facility staff worked through structured outreach, referral, follow-up and community dialogue.

Programme-reported results

119clients followed up in Quarter 1 and reported re-engaged in care
78clients followed up in Quarter 2 and reported returned to treatment
31clients followed up in April 2025 and reported returned to care

The report identifies retention and re-engagement as the programme’s strongest result. It also records expanded community testing, stronger referral pathways, counselling and psychosocial support, regular community sensitisation and improved community–facility coordination.

Lessons and next steps

Community delivery can overcome distance, mobility and stigma barriers, but testing should use more precise risk profiling and hotspot mapping to improve positivity yield. The report recommends strengthening digital tracking, transport and field supervision, workforce development and operational research.

Evidence note: these figures are programme-reported and have not been presented as independently verified. The report states an implementation period ending in March 2025 but also includes April 2025 follow-up data; both are preserved here transparently.
